These Terms & Conditions (the “Terms”) govern your access to and use of the Help Moms Transition website, platform, and related services (collectively, “Help Moms Transition”, “we”, “us”, or “our”). Please read them carefully before using our services.
By accessing or using Help Moms Transition, you agree to be bound by these Terms and our Privacy Policy. If you do not agree, you must not use our website or services.
Help Moms Transition is a connection and resource platform designed to help adults aged 55 and older, and their families or caregivers, discover and engage with local senior-focused service providers. These providers may include, but are not limited to, in-home care agencies, assisted living and memory care communities, senior real estate specialists, move managers, clean-out and downsizing services, and other senior support professionals (collectively, “Service Providers”).
Help Moms Transition is not a healthcare provider, insurance carrier, financial advisor, law firm, or real estate brokerage. We do not provide medical, legal, or financial services,. Our role is to facilitate introductions and provide information, not to deliver the underlying services of medical, legal or financial services.
By using our website, submitting an inquiry form, scheduling a consultation, or otherwise interacting with Help Moms Transition, you represent and warrant that:
If you do not agree to these Terms, you must immediately discontinue use of Help Moms Transition site.
Help Moms Transition may offer the following, depending on your location and individual situation:
The specific services available to you may vary based on your jurisdiction, health status, financial situation, and other factors. We reserve the right to modify, suspend, or discontinue any part of our services at any time without notice.
Help Moms Transition operates as an informational and referral platform. While we may have commercial relationships, referral arrangements, or advertising agreements with certain Service Providers, we do not control the manner in which they operate their businesses or deliver services. We do not employ, supervise, or manage Service Providers, and we do not guarantee their availability, pricing, licensure, insurance, or performance.
Any relationship you establish with a Service Provider—including contracts, consents, service agreements, payment arrangements, and care decisions—is solely between you (and/or your family) and the Service Provider. Help Moms Transition is not a party to those agreements and is not responsible for any disputes that may arise from them.
Each Service Provider is solely responsible for:
Help Moms Transition does not independently verify each Service Provider’s background, credentials, or regulatory compliance and makes no representation or warranty regarding the suitability, safety, or quality of any Service Provider or service. You are encouraged to conduct your own due diligence before engaging any Service Provider.
When using Help Moms Transition, you agree to:
You acknowledge that decisions regarding care, housing, finances, and legal matters for yourself or a loved one are significant and personal. You remain solely responsible for those decisions and any consequences that result from them.
If you are using Help Moms Transition on behalf of a senior adult (for example, as a spouse, adult child, caregiver, or professional advisor), you represent that you have the authority to share information and make inquiries on their behalf, or that you are acting in their best interests where they cannot act for themselves. You agree not to misuse the service to obtain information about, or make decisions for, a senior without appropriate consent or legal authority (such as a power of attorney or guardianship, where applicable).
Your use of Help Moms Transition is also governed by our separate Privacy Policy, which explains how we collect, use, disclose, and protect personal information. By using our services, you consent to our collection and use of your information (and, where applicable, information about your loved one) as described in the Privacy Policy, including sharing that information with selected Service Providers in order to facilitate introductions or consultations.
Help Moms Transition is not a covered entity or business associate as defined by the U.S. Health Insurance Portability and Accountability Act (“HIPAA”), and we do not process information in a manner that would make us subject to HIPAA. However, we are committed to handling sensitive senior-related information with care and in accordance with applicable privacy and data protection laws.
By submitting your contact details to Help Moms Transition (including phone number, email address, and mailing address), you expressly consent to being contacted by us and by selected Service Providers by phone, text message (SMS), email, postal mail, or other reasonable means, even if your phone number is listed on any federal or state Do Not Call registry. Message and data rates may apply for SMS communications, and you may opt out of marketing messages at any time by following the instructions provided in those messages.
In many cases, Help Moms Transition is offered at no direct cost to seniors and their families. We may receive compensation from certain Service Providers (for example, referral fees, advertising fees, or marketing support) when we introduce you and you choose to engage their services. This compensation does not increase the price you pay to the Service Provider, but it may influence which providers we present or prioritize. We aim to offer options that are aligned with your needs; however, you are always free to choose any provider, including providers not introduced by Help Moms Transition.
Any fees charged directly to you by a Service Provider are governed solely by your agreement with that Service Provider. Help Moms Transition is not responsible for billing, collections, or payment disputes between you and a Service Provider.
Content on the Help Moms Transition website, including articles, checklists, guides, assessments, or any information shared during phone calls or consultations, is provided for general informational and educational purposes only. It is not intended to be, and should not be taken as, medical advice, nursing advice, mental health advice, legal advice, financial planning advice, real estate brokerage advice, or any other form of professional advice.
You should always consult directly with qualified physicians, nurses, attorneys, financial advisors, real estate professionals, or other licensed professionals regarding your specific situation before making important decisions about care, housing, finances, or legal matters.
To the fullest extent permitted by law, Help Moms Transition is provided on an “AS IS” and “AS AVAILABLE” basis, without warranties of any kind, whether express, implied, statutory, or otherwise. Without limiting the foregoing, we specifically disclaim any implied warranties of merchantability, fitness for a particular purpose, title, and non-infringement, and any warranties arising from course of dealing or usage of trade.
We do not warrant that the website or services will be uninterrupted, secure, or error-free, that defects will be corrected, or that our website or any related systems are free of viruses, malware, or other harmful components. We do not warrant or make any representations about the accuracy, completeness, or usefulness of any information provided by or about Service Providers.
To the maximum extent permitted by applicable law, in no event shall Help Moms Transition, its owners, officers, employees, contractors, or agents be liable for any indirect, incidental, special, consequential, exemplary, or punitive damages, including but not limited to loss of profits, loss of data, emotional distress, personal injury, wrongful death, property damage, or other intangible losses, arising out of or in connection with your use of or inability to use our services or your interactions with any Service Provider.
In jurisdictions that do not allow the exclusion or limitation of liability for consequential or incidental damages, our liability shall be limited to the maximum extent permitted by law. In all cases, our total aggregate liability to you for any claims arising out of or relating to these Terms or your use of Help Moms Transition shall not exceed one hundred U.S. dollars (USD $100.00).
You agree to indemnify, defend, and hold harmless Help Moms Transition and its owners, officers, employees, contractors, and agents from and against any and all claims, demands, actions, liabilities, damages, losses, costs, and expenses (including reasonable attorneys’ fees) arising out of or related to: (a) your use of Help Moms Transition; (b) your interactions or agreements with any Service Provider; (c) your breach of these Terms; or (d) your violation of any law or the rights of any third party, including any senior individual on whose behalf you act.

We may, at our sole discretion and without prior notice, suspend or terminate your access to all or part of Help Moms Transition for any reason, including but not limited to: (a) your breach of these Terms; (b) suspected fraudulent, abusive, or illegal activity; or (c) our decision to discontinue or materially modify the platform. Upon termination, your right to use the services will immediately cease. Sections of these Terms that by their nature should survive termination shall continue to apply, including but not limited to those relating to intellectual property, disclaimers, limitations of liability, and indemnification.
We may update or modify these Terms from time to time in our sole discretion. When we do, we will revise the “Last updated” date at the top of this page. Changes will be effective when posted unless otherwise required by law. Your continued use of Help Moms Transition after any changes to the Terms constitutes your acceptance of the updated Terms.
